To provide you with the best possible care, It is important for your Clinical Dental Technician (CDT) to have your medical history and understand your health needs before any examination or treatment is carried out. Without an up-to-date medical history we will be unable to provide any treatment.

 

All information we collect will be kept strictly confidential and handled in accordance with our General Data Privacy Notice, a copy of which is displayed in practice.

Consent is required by Smile Denture Clinics to carry out treatment and process your personal data, but it must be explicitly given. Where we are asking for sensitive personal data we will always tell you why and how the information will be used. By submitting this document you are consenting to our privacy policy and you are giving us permission to process your personal data specifically for the purposes identified in the privacy notice.
